MedicalHistoryAI is an automated medical document organization, indexing, and summarization tool. It is NOT a medical device, diagnostic platform, or healthcare provider. It does not diagnose diseases, prescribe pharmaceuticals, or provide clinical advice. All AI summaries are informational aids that must be independently evaluated by a licensed healthcare professional.
You are responsible for safeguarding your login credentials and ensuring the legal right to upload any medical documents submitted to your account. You agree not to upload malicious files or attempt unauthorized access to other accounts.
When you generate a share link, you authorize the holder of that token or passcode to inspect the designated medical summaries, timeline, or documents. You may revoke active shares at any time from your Patient Portal.
Paid subscriptions (Patient Pro, Doctor Practice Pro, Clinic Team) are billed through Paddle. Subscriptions auto-renew until cancelled by the user. You can manage or cancel your subscription at any time via the billing portal.
To the maximum extent permitted by law, MedicalHistoryAI and its operators shall not be liable for any health outcomes, diagnostic omissions, or medical decisions resulting from the use of AI-generated summaries. Use of this service does not establish a doctor-patient relationship.
